Adam Jackson, the lead class designer, shared additional explanations on his Twitter page to describe the unique items in more detail and clarify the conditions for finding them.

The six items in question include:

  • Doombringer (unique sword)
  • The Grandfather (unique two-handed sword)
  • Ring of Starless Skies (unique ring)
  • Andariel’s Visage (unique helm)
  • Harlequin Crest (unique helm)
  • Melted Heart of Selig (unique amulet)

According to Jackson, each of these items can drop from enemies that are at least level 85.

There is no specific location to find them—they can drop anywhere normal unique items can be found.

Jackson mentions that you can identify one of the rarest items by its item power of 820, but he also emphasizes that "they're really rare," indicating that the chances of finding them are extremely low.

Diablo 4 was released on June 6, with early access available from June 1. These six items are so rare that one of them, the Harlequin Crest, was only discovered on June 10, more than a week after the early access started.
